Assumption of Risk

By signing this waiver, you acknowledge that participation in fitness and wellness activities involves inherent risks of injury, including but not limited to muscle strains, sprains, and other physical harm. You voluntarily assume all risks associated with your participation.

Release of Liability

You hereby release, waive, discharge, and hold harmless [Fitness and Wellness Company Name], its employees, agents, and representatives, from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by you while participating in any activity, or while on the premises, whether caused by negligence or otherwise.
